Business Information to Provide
- Name
- Address (mailing and physical)
- Phone Number
- Email Address
- Tax Identification
- Nature of Business
Information Needed from Each Business Owner and Authorized Signers*
- Name
- Address (mailing and physical)
- Social Security Number
- Date of Birth
- Primary and secondary identification *All owners/authorized signers of business must be present at account opening
Sole Proprietors
- SSN or EIN (Employee Identification Number information at www.irs.gov) may be used as the Tax ID
Corporations
- EIN (Employee Identification Number information at www.irs.gov) may be used as the Tax ID
- Articles of Incorporation as filed with and stamped by the Secretary of State or the appropriate state official
- Fictitious Name Certificate or Certificate of Trade Name (if doing business under another name other than the corporate name)
- If a professional corporation, a Certificate of Registration from the appropriate licensing board
Limited Liability Company (LLC)
- EIN (Employee Identification Number information at www.irs.gov) may be used as the Tax ID (if there is only one owner of LLC, then you may use owner's SSN)
- Articles of Organization as filed with and stamped by the Secretary of State or the appropriate state official
- Operating Agreement if more than one owner
- Fictitious Name Certificate or Certificate of Trade Name (if doing business under another name other than the LLC's name)
- If a professional corporation, a Certificate of Registration from the appropriate licensing board
